We accept most insurance plans, including Medicare, Humana, Aetna, Blue Cross/Blue Shield, third-party insurance, workers’ compensation, and many others. We will file your insurance claim for you.

Here are a few things you should bring with you to your appointment:

• Your insurance card. We will make a copy of it for your patient chart.
• Co-payment. Most insurance plans require co-payment at the time of service. Our office accepts cash, checks, MasterCard, VISA, American Express and Discover.
• Any changes in your address or telephone number.

If you are not insured or need to meet a deductible, our staff will be happy to work with you to make payment arrangements.
